Entry Form Instructions
ONE FORM IS REQUIRED PER ENTRY: Make photocopies of the Entry Form, complete all information and attach to entry.
Preparing your own entries: An individual submitting his/her own work should provide information under Author of work and Media in which work appeared/aired, including name of applicable editor/contact. For Preparer’s name, put “same.”
Preparing entries for others: Media preparing entries for staffers and/or free-lance journalists should provide applicable information for both the preparer and the Author. For free-lance work, under Author give the author’s direct contact information, not the information for the media office.
Newspaper Travel Sections: When designating category number (2A or B), provide current circulation of your publication for the day of week in which Travel section publishes. Check your circulation to be sure your section is in the correct category.
Entry data: Fill out applicable portions below for your entry. For categories in which you may submit more than one work as an entry, such as Grand Award, attach a sheet to form, listing headlines/media for all entries.
Mailing address: Send your packet with entries and a check made out to SATW Foundation, or a receipt for credit-card payment through PayPal on our Web site, to:
